The type of personal information we collect
For people referred to our Hospital Services, we may collect and process the following information:
- Name, date of birth, and NHS number (where provided)
- Contact details (address, email address, and telephone number)
- Demographic information such as age, postcode, preferences and interests
- Health or care information relevant to your referral and support needs
- Information about your hospital admission and discharge
- Other personal information relevant to the support we provide to you
How we get the personal information and why we have it
Most of the information we process is provided to us in one of two ways:
- Directly by you, when you register with Carers’ Resource for support
- Indirectly, when a health or social care professional refers you to us (for example, as part of hospital discharge planning)
How we use your personal information
We use the information you give us to:
- Contact you to discuss the support available from Carers’ Resource
- Provide you with the service you have asked for or agreed to receive
- Record the outcome of a referral (for example: accepted support, declined support, not eligible)
- Share required information with NHS partners so they can monitor outcomes, including whether people who have been referred are re-admitted to hospital
- Provide anonymised reports to our funders about the range of support we offer
- Improve the quality and range of services we offer
Sharing your information
- If you choose to register with us, we will create a full support record in our secure database (CiviCRM).
- If you decide not to register with us, we will still hold a minimal referral record so that we can meet our contractual duty to report referral outcomes to the NHS. This record will only contain the minimum details needed to identify the referral and outcome.
- We will never sell your information. We only share it where required for the purposes above.
Our lawful basis for processing your data
Under UK GDPR, the lawful bases we rely on are:
- Consent – when you agree to register with us and receive support services.
- Public task – where we are working in partnership with the NHS and required to record and report referral outcomes.
- Legitimate interests – where we need to hold minimal data to evidence that a referral was received and responded to.
How we store your personal information
We are committed to ensuring that your information is secure. We keep personal information relating to referrals and support securely in our database.
- Referral records (including declined/closed referrals) are kept for seven years for adults, and up to the age of 25 for children.
- After this time, we will securely delete your information in line with UK GDPR requirements.
Your data protection rights
You have the same rights as set out in our main privacy policy, including the right to access, rectify, or object to how we process your data.
